As pediatric patients with celiac disease transition to adult care, maintaining continuity can be challenging. This session explores best practices for successfully transitioning care from pediatric to adult GI, with a focus on maintaining continuity, patient trust, and long-term outcomes. The event will also feature a patient advocate who will also highlight the patient perspective, sharing what individuals with celiac disease need, value, and expect from their health care professionals as they move into adult GI care.
